I used to work for Pacific Bell (Which became SBC and is now AT&T again). The office I worked at housed the switching station for all telephone lines in the area code. A number of times a week law enforcement of all levels was in our office, installing equiptment and basically tapping phone lines.

So telling me that the NSA showed up at the AT&T offices means nothing to me. As for the documentation, yes, I'm sure there are such documents saying "this is how to tap into a phone line using this system" because it's done on a regular basis.

Being as I don't talk to terrorists and don't do anything illegal, I could less who in the government has access to my phone records. What worries me much more is the fact that anyone can purchase records of my transactions and payments to banks and credit card companies. This is already perfectly legal.
