Depending on what you want to do, Skype so far has been the best solution for what I am doing with it. You pay about $30 a year for an incoming phone number, and pay about .02 a minute for all of the U.S., Canada, Europe, Australia, by buying pre-paid time in $12 blocks.

About 90% of my long-distance calls are with Skype, and soon that might be as high as 98% with the new VOIP phones from Netgear and Skype which will use 802.11.
