Steroids have anabolic effects on other parts of the body besides muscles. 5-alpha-reductase converts testosterone to dihydroxytestosterone, which is responsible for external male sex organs. People lacking 5-alpha-reductase but that have XY chromosomes (Jamile Lee Curtis) don't have male external sex organs but are genetically male.
