He was big, certainly the 3rd most known adult publisher in his day, but a lot lower profile than Hefner, and from my recollection Flynt. (I'd guess Penthouse's circulation exceeded Hustler's, I'm just talking about the profile of the publishers). Hefner and Flynt both promoted themselves as a way of promoting their magazines, and in Hefner's case his brand (Playboy Clubs, etc.), while Guccioni was far less visible. Hefner would get regular tabloid coverage just as a general celebrity, and Flynt would do/say controversial things to get in the news regularly. In fact they still do, just with less intensity/frequency as in their heyday.
