It is just a basic form, you only need to purchase an SSL certificate for the server. You can install it yourself or your server techs should handle it for you. No programming is needed if the form is ready.

Once the cert is installed, you simply call it from https://etc...

You can get certs from many places...

http://www.verisign.com/
http://www.thawte.com/ (I believe ccbill uses them)

For credit card processing do not go with anything less than 128bit.
