they don't incourage these people to go out and have unprotected sex...they simply tell them to go on about their lives.

Actually, the first phase of testing simply consists of watching the effects of the vaccin itself. The problem with a vaccin is that it could, if the virus hasn't been weakened enough, infect the person receiving it with the real disease.
After that, they'll probably look at the direct effects on the immune system, and only after that would they go into large scale (thousands of people) real world tests.
