At a coroner's inquest/inquiry, what happens is they examine all the factors that contributed to the death and try to come up with ways of ensuring the same situation doesn't happen again. Just so you know, these types of investigations are NOT concerned with assigning blame to a murderer, more like just finding out what other factors were involved.
