Overture and other search term inventory analyzers are a good place to start.

for ex:

"medical" is the 5th most popular associated term with the word "fetish" and i dont see many medical fetish sites out there.

Also search yahoo groups (or other community based sites) for obscure fetishes with a lot of activity/members - that can also be another good early indicator.
