Is fisting, hardcore facials, or urination illegal in the USA

Or is it just considered 'obscene' in some states? I remember seeing a show talking about Larry Flynt signing off on these things saying he won't produce this type of content but I don't remember if it was considered illegal or just possibly obscene.
