What they do is they shove a little metal rod thing up your urethra. and then bend the tip of your penis upwards at a sharp angle so that they make sure they get the right spot, and then they poke the needle through!

And OH does it ever bleed.... there's a lot of blood vessels in the penis for the purpose of erection, so it takes a long time to stop bleeding
