Have a form on your site ( pdf or other ), get the surfer to print and fill it by hand, join a copy of his driver's licence. Send the user/pass to the address on the drivers licence.
Not many will do that tough, but if you do and it's a minor, you at least have the defense that you took extra steps to confirm his age.
