We've had this happen before, and of course it's not on purpose on our end. Usually this will happen with programs that use CCBill since sometimes you can't tell what program you're signing up for exactly. It's also pretty common on programs that have brought in other individual sites, since we think we have to sign up for their new program (when in reality they're now just part of a program that we already list, and we just didn't realize it). We get emails daily asking us to switch aff codes, sign up for their new program, go back to our old codes, etc. Other times, it's just an honest mistake with someone on our end glossing over the fact that we're already an affiliate there.

I think a simple email explaining it works fine, or else provide some way not to sign up twice inadvertantly (based on SS#, EIN, ESP, blood type, whatever).
