With all due respect, you know your stuff man... but this does happen sometimes. In large corporate environments, some companies fail to coach and develop a successor for their CEO.
General Electric (GE) is known as THE company that breeds the best managers in the world, and this is how the manage to have so many Business Units. Some other companies - those with CEOs that have bigger egos than love for the long term life of the company, bigger than a human life, forget about this detail.
When they get old, they don´t have the time (or patience) to breed their sucessor, so they either hire someone from another company, or they buy a smaller company where the profile and style of the CEOs match. So yeah.. this can happen.
