To me hand and wrist jewerly always has been a distraction. Nothing perturbs me more than a masturbation scene where a girl is wearing a damn watch.
As for people noticing well I am sure many do not, but when members can leave comments about any given content like they can in my sites it only takes one person to notice and then wham everyone does.
