The fact is that most really do it by bandwidth. The "per member" deal is really archaic since it was done that way when everyone worried about hacks that would allow way too many in and webmasters would be paranoid about having to pay the bandwidth for all the freeloaders.
In actuality, these days if you are paying "per member" prices then you are most likely paying way more than you would on a well priced bandwidth plan
(did I hear someone say Homegrown Video..?) A good bandwidth plan means you are paying for exactly what your members are using and most content companies worth their salt have created security that prevents the unwanted viewers... unless of course you are letting in cracked passwords intentionally... but that is another story.
A good price on bandwidth fee plugins is probably less than the bw most people pay for the rest of their hosting because if they are successfully established to bulk buy enough bw.
