I'm no expert, but I think a lot of it has to do with lifestyle too. Addicts usually don't eat very much because they lose their appetite and prefer to spend money on the drugs instead of food. They usually wind up homeless and the elements outdoors wreaks havoc on the body. With prolonged use I'm sure the drug itself causes it's own problems too.
Sad situation any way you look at it. But it is a choice they make.
