I'm not sure where you live, or how they do it there.
With my insurance, you make an appointment, and if you want to see your doctor, then you might be looking at a week or two wait. If you want to just see anyone in ProMed then they can normally get you in same day, or next.
As for payment, they expect the co-pay at time of appointment, and do all the rest in regards to sending in your insurance shit for the other $70 of your $75 visit.
