If you have the money, Cisco's VoIP solution rocks.
The large telcos are already handling a lot of their "long haul" voice over VoIP. By doing that, they don't have to pay tariffs to the Govt. like they do with traditional voice lines. I mention this because for the end user, it means easier access to VpIP technology and more provider choices. VoIP requires a gateway at the LEC, for both origination and destination of a VoIP call from a regular phone.
Call some phone companies and see what they offer before you make a choice...
