My escort service is in the UK, where our currency is stronger than the USD, yet I still have women selling their bodies for sex. All seem happy with the arrangement, who are you to say who is being exploited?

If they're indeed doing it out of their own free will and have economic alternatives, there's nothing wrong with it.
On the other hand, even in my first world country, a recent study found that a fifth of all prostitutes are being forced to do the work. It doesn't seem particularly odd to assume that in third world countries that amount would be much higher, and in that case, abusing that situation could well be considered immoral.
Furthermore, the mere fact that people are smiling and look happy at a certain moment while doing their work does not mean that the situation is morally right. For example, when slavery was legal, one can safely assume that at times, slaves would be laughing, smiling or singing while doing their work. Nevertheless, they were being exploited at the same time.
