RewriteCond %{HTTP_REFERER} ^.*usefulidiot.*$ [NC]
RewriteRule \.(jpe?g|gif|bmp|png)$ /test/hotlinkimg-d.gif [L]
There should be an OR in all the RewriteCond's except that last one.
