Nathan, thanks for the clarification. I'll make the changes in the final HTML output, but if you want me to, I can add the form elements to the screenshot.
Additionally, the colors can be swapped out (meaning, the red scheme replaced with a blue scheme and so on). So the color scheme can be easily matched with the base color scheme of the affiliate program's main website.
