I usually take a washable marker and draw a circle or oblong or whatever pattern I may have in mind on the back, (or front, whichever), of my willing victim. Sometimes it takes an entire box of needles, and sometimes it only takes maybe 20 or 30, depends on the design I'm after.
Insert needles in their proper places in the pattern. Very carefully wind the colored twine around the needles to produce the final result. I'll dig up some pictures of some pretty spirograph stuff.
