Well, we only have a PPS program, which is why I don't have to worry about VISA, however, our affiliate that makes five figures every month has a lot of content on his site.
Some of the others that seem to get checks every two weeks are equally large, and in fact one used to be a paysite until CCBill EU told her they would no longer process for her. She changed her join page to ours (nothing else) and went from 1:1500 to 1:20 over night.

So, while we do have a lot of standard sites in the system, the people that submit premium style are doing quite well.
In fact one of my affiliates posted this
tutorial here a couple weeks ago.
He has a lot of content, and makes lots of sales every day.
You might consider mirroring your site. Do one version with a premium and get the recurring, and do another for the easy, quick money
