I used
http://www.delawareinc.com and was satisfied with it. I think it was about $1000 - $1500 but that was in 1998. The only thing I didn't like was that when I wanted a bank account opened there in Delaware, they charged another $500 to do that (which wasn't made clear to me initially - but like I said, that was 6 years ago).
