I have to disagree with both the shoulder holster and the small of your back thing. While both are comfortable, most of the time, they both have problems.
The shoulder holster sucks when you get warm. You can not just shed your jacket while in the mall. Also, as K-Man pointed out... grabbing through the layers can be a pain. In most cases you are not very calm when you have to reach for your piece. Trust me on this.
Now, the back thing. This is bar far the most comfortable place and way to carry. It was my choice for a long, long time until I was in a bar fight and I watched a friend get picked up and thrown down onto a pool table. He had a small S&W 9mm tucked in his back and when he hit the table, it pushed back into his spine. He was in a wheelchair for almost 2 years and still has trouble walking today.
After seeing that, I switched to a paddle holster and carry inside the pants on my right hip. The holster clip is small and blends in with the cell phone. I usually carry a Sig P228 which is a full sized auto and I have no problems concealing it even with just a t-shirt.
