fill out the form...they tell you you'll have to send medical records, you don't, it's BS. Put a doctor name and number in the "who's your physician" spot (they don't have the time to call)
Say you have lower back sciatica related pain that acts up once in a while.
A doctor will call you in a day or so, tell him the above, you were at a doctor about 6 months ago, but you have no medical insurance anymore. tell him you have medical records, a cat scan, and x-rays, he'll give you a fax number but push the order through anyway in the mean time...no prob.
He'll write you a good prescription, forward it to a good RX and fed ex it right to you're door. no laws broken...you have a right to medical attention.
