I use IE6 (patched of course) with security settings on maximum high for all but sites that I manually add to the list of Trusted Sites. This eliminates 99% of the known vulnerabilities, especially those fucking ActiveX security holes that you can fly a fucking 747 through. And then Ad-Watch set to start at start-up helps too (so nothing changes that fucking registry without my say-so), and regular Ad-Aware scans and updates too.
Don't rely on Ad-Aware solely - it won't work on the new and nasty stuff that is out there - you've got to block the shit at the source.
The downside of having ActiveX and other browser features completely disabled is I don't get to see the videos most of you guys post, and a lot of other stuff on the net that is on sites that I don't completely trust. But it's better than letting some asshole fuck up my machine.
ps. There is a RealPlayer vulnerability you should make sure you have patched too - it allows malware to be installed unknowingly. Click 'Updates' under 'Tools' in RealPlayer.
