You might ask you vet to consider 'pulse' treatments in which you regularly give the dog treatments for the infection (antibiotics and ear drops) even if the dog doesn't show signs of infection.
I have an older cat that is prone to respiratory infections. He gets 10 days of antibiotics every month to help prevent them.
