Well the sucking action will make the ear hurt.
Maybe you could make the nipple hole slightly larger (the one that goes on the bottle for God's sake, lol). But be careful not to make it too large (choking hazard).
Ear problems can be hell... I popped a hole in my eardrum while diving and I thought I was gonna lose my mind.
