Frodo is leaving cause he is dying. The stab wound that he got in the first movie is killing him. The elves, Gandalf, Frodo and Bilbo are leaving Middle earth. Well, Frodo is not dying, but he cannot handle the pain anymore.
That's why we saw him complain that his wound still bothers him and it will never heal.
But the ring also affected him and the fact that he almost betrayed everyone with it.
The only peaceful place left for him is with the elves, the emotionnal and physical pain will be gone.
