If they don't want the money they can sign a form and give it to charity. No big deal. If doctors really wanted to do the right thing, they could do it for about 1/2 the price and cheer everyone right the fuck up. The problem of course is that Ferraris ain't getting any cheaper and golf course memberships don't grow on trees.
