Unfortunately this error plagued me for DAYS! I even reinstalled my operating system hoping that might have been the problem. I installed Photoshop and imageready many times too. I spent those days troubleshooting hardware, software, and everything in between trying to find what caused the conflict.. Unfortunately I am pretty sure it's an actual error in PS, however certain conditions have to be met to encounter it. Once you do, it doesn't go away again. I don't understand why. But anyways, this is actually the first time I've ever encountered anyone else having the same problem. Everyone always thought I was fucking crazy
I'm relieved to know I'm not alone nor crazy.
