DMCA the payment processor, registrar, web host, etc.
Perhaps those who do this regularly can comment, but I always felt directly DMCA'ing the site owner is a waste. It may be the fastest way to get links down, but there's no real pressure where it counts the most.
On the other hand, getting an email from your source of income telling you to take down the content...
