If I want a model to produce a video for me so I can put it on my website for public viewing with a watermark (my site name) and have it exclusive. How do I go about it: where can I find a release form template she can sign and do I need to consider anything else?

What if instead of a public video I want to bill customer as part of a video on demand service, any template for that too?
