In what? I've been to Russian free hospitals in 80's and in 2000. And it was not in Moscow (about 1400-1500 kilometers to the East). Maybe something was uncomfortable, like about 8 persons in the same room, no lcd panels and no wifi, but everything else was ok. I've got all the treatment that I needed, the free meds (including tranks), various tests, mri etc. Even the food is free there. And no, both times there was no dirt. They are washing the rooms with a bleach and doing ultraviolet irradiation daily. That's very simple measures, but very effective. So I don't just believe in that. I've been there and saw everything by my own eyes. And I'm not blind.
And once again, if you don't want a free one, pay the money and go to a private clinic with blackjack and hookers. At least you have a choice, right?
