No.. I knew of a few ACH processors years back and there was no setup fees - also the one's prompted on this thread have nada setup costs.
BTW ... It may be a clue to use "non standard" (ie. not IBill, CCBill, 2000Charge, proBilling etc) ACH processing services to split the risk in having any one processor hold volumes of recurring revenue
