There are a few issues that I can think of off the top of my head.
In each state, there is a marketplace where you can buy insurance. How many different companies are offering plans and how many plans there are to choose from can vary greatly from state to state. My state has a ton of options so the plans are affordable, but there are some states that may only have one choice so they can be very expensive and may not suit your needs.
Another issue is Medicaid expansion. Obamacare is setup in a way where states can expand their Medicaid coverage to more people (mostly poor and sick). The problem is, there are a number of states who have refused the expansion. This means there are potentially millions without coverage.
Speaking of Medicaid, it can also be a problem in and of itself. There are a growing number of doctors who refuse to accept Medicaid patients which means those patients may end up going to the emergency room or urgent care center for treatment. This causes costs to rise and clogs up the ER and urgent care centers.
Mostly, however, the biggest problem is cost.
