The comment "you are disgusting" is being turned around on him. He could have meant it in the context that the lawyer was disgusting because she had no tact or class by simply whipping out the breast pump in court.
In no way was he condemning the actual act of breast pumping to feed the baby.
It's the manner in which the lawyer did the request which drew the ire of Donald.
