I have been using a model release waiver for all my models acting/fucking in any shoot. It's pretty much based off of an example I found online a while back. I'm getting ready for a bigger shoot and trying to make sure things are as legit as possible.

Does anyone have any examples of a good model release waiver. Also, a contract agreement that goes over the payments for the model and how they will be paid depending if they are in a certain agreement like they get 5% of all profit generated by a specific DVD, video, etc..
