I fight all the time too, and watch my drunk stupid friends get in fights even more... and I rarely see a fight go to the ground, not even if it's only 1 person going down... but I would say that 95% of all fights end up in a clinch at least once, so in that sense, grappling is involved.
When people fight in bars and such, they typically don't grapple or go to the ground. They are angry, their mentality is to want to punch and hurt each other, not roll on the ground.
I have trained BJJ for 4 years, and I have come to the conclusion that the best ground defense is to just get the fuck up fast. The popular misconception of "90% of all fights going to the ground" is made up by the Gracies, and for them, it's true because they take all their fights to the ground on purpose.
