I'm not really sure how to answer that. You might be reading more into it than is necessary. I don't really think of it as "designing" the shot in my head. The individual shots take 2 seconds to shoot. Not much thinking involved. However the set up of a set may take anywhere up to an hour to think through and prepare, particularly if it's an indoor set possibly with an outdoor background and multiple areas to light. If it's a sunset then you just gotta bang it out before you lose the conditions or else anticipate the lighting set up in advance. If you factor in the location then I suppose I am designing the set way in advance because that takes months to research, book and prepare for.
