the problem usually starts when you dont have money and you buy crap that has all kinds of stuff mixed in there - up to rat poison and similar

I would not fully agree with this statement half i think is true but the other half isn't, I will explain here.
People mix heroin to make it weigh more like the guys selling it the more crap they mix it with the heavier it is and the more money they can get for a smaller amount of the actual drug.
When snorting heroin they are snorting all the crap like the rat poison you mentioned, but on the other hand when injecting heroin they put the heroin on a spoon and burn it this results in the crappy stuff being burned away and only leaving the purer heroin as it does not burn away as quickly as the crap they have "chopped" it with, when burning they use something like Lemon juice to help break down the heroin, they also normally suck it into the needle via a cigarette filter or something to again filter out any of the crap.
So as silly as it sounds when "snorting/sniffing" heroin it is actually more dangerous as in with regards as you do not know what you are actually putting into your body, it is actually cleaner for the user to inject. All in all most users end up messed up as they do not do it in a clean way and they share needles etc and a lot of the time they are in such a rush to get there next "hit" they can actually ruin their bodies by not preparing it correctly or by the simple fact that are injecting into parts of their body like the groin etc where needles simply shouldn't be used but the user is left with no choice as their Veins are all fucked up and collasped.
I have heard of guys that have had to inject themselves with Rat poison (Warfarin) to thin their blood where they have blood clots from the reason explained above.
All said and done it is a evil drug and responsible for thousands of deaths and crimes daily..
They are finding better drugs to combat the addiction now, methadone to a user is just free heroin and will never be a proper medicine as most users will use the methadone and still get heroin too,
