Didn't read it all but it looks confusing!
Too many buttons. Try to cut down. ie pay by echeck should be alternate payment with the telephone, not up with CCs. Thats where I would put it anyways.
Also all the shit at the bottom. Not sure what your processors require but id put one support button that then had all the support options in it. Idk if they require it like that though.
FYI: I do not run an adult paysite
