they charge for toll free (888,877,866,855,800) numbers in the US. Most places are $10/mo or so while others you buy the number but need to "host" it somewhere.

the provider will charge you $.02 to $x.xx per minute for incoming calls or give you a plan with xxx minute then $x.xx after. forwarding the number form a US toll free to UK can get hellishly expensive, upwards of $3.00 per minute.

if you are looking to do US toll free to UK, I would do US toll free to local number, such as a plan with vonage, then forward that to a UK land line where you don't have to pay for international calls. If its going to a UK phone, then forward the UK land line to a cell. Confusing, but will cost you very little to do. I had to do that for a company in New Zealand a few months ago
