I would hazard many do realize they're breaking the law, but justify it as ok for various reasons ... everyone else does it; it's already out there; it's just computer bits; website has ads on it; i already pay ISP fees; etc.
Furthermore, in the minds of most surfers, the perceived "value" of adult content, and likewise with most mainstream content, is virtually zero - it's just a commodity that's not overly unique and hence not worth paying much, if anything, for. Not saying that's necessarily true, but that's the perception by most.
Ultimately, it comes down to the bottom line ... download sites are far more cost effective for most people than separately joining numerous sites; not only in dollars and cents, but also in time and hassle - it's easier to join / cancel membership at one file sharing site verses dealing with lots of different adult sites directly.
