I switched out my captcha to use q&a captcha and it stopped the spam shit dead in the tracks.
I also have some custom mods that I made to the registration to kill users coming off of ripe or apnic ips. Since my forum is supposed to be a local forum it works for me.
I used to send anyone I could determine was a non-local user to a playpen of sorts where they could spam away but noone could see it but when I hit about 10k users in the playpen the hits against the db weren't worth the fun of watching them spam themselves anymore.
If you did want to change out the links it would be easy enough to run a script through the posts and change out all the urls in the posts, only problem there would be identifying urls in legitimate user posts so that you don't change them out.
I don't think you could do anything in htaccess because the links point to other servers, to redirect them you would have to have access to the links destination servers to do that. (Key phrase in there is "I don't think" ;p )
