When it comes to assisted suicide I think it means that you choose to end your life before things get really ugly. Sure, the moment of death is not dignified, but I think they refer to people who have ugly debilitating diseases where you will slowly wither and die, have to have people wipe your ass and shower you and you end up not knowing who anyone is as being a loss of dignity.
