I grew up in London and lived in Manhattan for 4 years before I moved to SF, and yes, compared to those cities, SF is small. For one thing, it's population is 700,000 compared to 7-8 million for both NYC and London.
After living in SF, I'd say small is a more of a state of mind than the size or population of the city.
