Instead of the fittest and strongest people breeding and the ill and defective dying young modern medicine means everybody gets a go at having kids. Hereditary diseases that should kill people before they reproduce are treatable so instead of the disease dying out with the person who has got it they have kids who are either going to have it or carry it to reappear again in a future generation. Eugenics is the control against this.
