Parents, cell companies, and apple wouldn't want to continually spend the time to update and manage the software and controls to ensure users must be 18+. It's just too much time and money. Where there is a will, there is a way - it wouldn't take long for people to get a work around getting those 18+ apps on <18 year old phones. It's just more efficient to ban them straight up. As well as it would piss off a lot of customers if they suddenly threw porn apps onto the app store.
